Required Documentation

Only completed application packages submitted by the deadline will be considered. All application materials must be submitted in one application package. Complete application packages must include the following:
- Two letters of recommendation
- One Letter of Recommendation from a member of the faculty at your high school.
- One Letter of Recommendation from a community organization (i.e. faith-based; civic; Boy/Girl Scouts; Boys & Girls Club; Jack-N-Jill; pageants; etc.)
- The most recent official transcript with the school seal and/or the signature of an authorized school official.
- An essay explaining what inspires you and how you hope to inspire others. How will your education allow you to make that happen?
- A mailed or electronically submitted Phillips Scholarship Program application. The application must be completed in its entirety. Address if needed:
